This simple form is part of a real system and is used to record the date, source and enquiry details for the incoming phone response. The full details of the enquiry are recorded in the note and categorised record in the Table User defined field. The latter enables an analysis of common concerns about the product offering so that it and the marketing presentation of it can be improved.
